Just fill in the necessary information (see below) Sections to fill out:

Location
All of PART 1 – Student’s Personal Information
All of Part 2 – Student’s and Parent’s Certification
Only High School Unweighted GPA in Part 3.
ONLY FOR FIRST TIME DUAL STUDENTS -> Only fill all applicable sections: PART 4 – Alternative Placement Criteria Eligibility.
For "Test Type" section, choose PSAT only.
Don't click "Edit", you can already type in the PDF when you open it. DO NOT SIGN AND DATE THE FORM.
MDC will request for you to sign it later via docusign (check your email).
After you finish filling out the form, save the file through adobe acrobat. To save the file: Press File (on top left corner of adobe acrobat)-> then press Save As.
After you save the file. Make sure to email Ms. Olivert @ [email protected]
After you fill out the DE form, email [email protected] and attach your DE form including your MDC course history. Ms. Olivert will schedule an appointment with you to go over your dual enrollment registration.
